As of February 2003, there are two printings of the book.

In the second printing (identified by the numbers 10 9 8 7 6 5 4 3 2 on page iv), we have corrected all the errors that were identified in the first printing. We are very grateful for the efforts of many kind readers in pointing out the flaws.
